Organizational Development and Its Role in Enhancing the Efficiency and Effectiveness of Health Services in Crisis Settings: A Comparative Applied Study of Yemeni Hospitals in the Capital Secretariat of Sana’a

This study aimed to measure and test the effect of organizational development, through its four dimensions—systems and policies, methods and technology, training and development, and analysis and adaptation of the organizational structure—on improving the efficiency and effectiveness of health services in Yemeni hospitals in the Capital Secretariat of Sana’a, with a comparative analysis between the public and private sectors. The study adopted the descriptive-analytical approach to diagnose the phenomenon under conditions of crisis and resource scarcity. A proportionate stratified random sample of 400 individuals was selected, from which 347 valid questionnaires were retrieved for statistical analysis, representing a response rate of 86.75%. The statistical results, using SPSS v26, showed that the overall level of organizational development was high (M = 3.94, SD = 0.64), with the systems and policies dimension ranking first (M = 4.16). The overall level of health service efficiency and effectiveness was also high (M = 3.92, SD = 0.71). Multiple regression analysis revealed that the dimensions of organizational development explained 55.8% of the variance in health service efficiency (R² = 0.558), with systems and policies being the strongest predictor (β = 0.315, p < 0.001). They also explained 48.6% of the variance in health service effectiveness (R² = 0.486), with methods and technology emerging as the strongest predictor (β = 0.341, p < 0.001), followed by analysis and adaptation of the organizational structure (β = 0.308, p < 0.001). In contrast, the analysis revealed no statistically significant direct effect of the training and development dimension in either model (p = 0.054 for efficiency and p = 0.857 for effectiveness). This finding was discussed in terms of the phenomenon of the “training paradox,” which may be attributed to the randomness of humanitarian aid programs, the migration of qualified personnel, and the deterioration of the physical operational environment. The study also identified statistically significant differences in favor of private hospitals, respondents with higher educational qualifications, and those with moderate work experience (4–7 years) The study concluded by proposing a guiding model and a set of practical recommendations to support institutional recovery.
